Table and Form Assignment

The purpose of this assignment is to use Microsoft WORD to create a table to organize information and a form to collect information about individuals within a program.

  • Create a table that fits on one page and can be used in a program to collect information.
  • There should be a minimum of 15 fields of information. Each field must be appropriate to the information or data that it is to contain (size- height, width, etc.). As appropriate, the fields should be labeled with headings.
  • Include the folowing elements within the table itself: vertical text, horizontal colmn span, vertical column span witih enlarged text within the column, table cell with a fill inside of it, table cell with a changed border, and two different fonts.
  • Additional requirements include a WordArt graphics at the top of the page and an autoshap and textbox located somewhere on the page.
  • Once you have finalized your table, print a copy of it.
  • After you have printed a copy, covert the table to a form.
  • The form should include a text form field field, a check form field, and a drop down form field. The type of form field should be appropriate to the data being collected. When using the text form field, it is helpful to specify the length of the text that can be entered, otherwise it may disrupt your form.
  • Once your form is completed, lock it and save it. Then have a classmate use the form and enter the required information (note that you may need to be creative here and make up some of the responses).
  • After entering the information, print the form twice --- once with the form and the data entered and once with only the data being printed.
  • Hand in three printed documents -- the table, the form with data, and the data only printout.
