Special Housing Accommodation Request Form

Special Housing Accommodation requests are reviewed by Academic Support Services in consultation with health services, counseling center, or other appropriate college offices. The goal of this review is to provide an on-campus housing accommodation that best meets the need of the student. Campus housing options may be limited based on space availability.
