Early Alert Form

IC faculty are encouraged to complete the following information regarding students who may be in academic jeopardy in specified courses.  Academic Enrichment Services will reach out to the student via email, invite them to an individual appointment and offer academic support services.  We will also make referrals to faculty advisors and other campus offices/staff as needed.  Complete the information below and send electronically by clicking "Submit".  We will ensure confidentiality of student information.  Also, please feel free to email us and we will promptly respond!
