GERONTOLOGY INSTITUTE ANNUAL CONFERENCE

"From Silos to Systems: New Health Care Models for Older People"

October 4, 2012

Conference Overview

Current massive changes in the health system, propelled by the need to reduce costs and improve quality, are bringing medical care and community health closer than ever before. Longstanding community health concepts such as prevention, disease management, and community-based strategies are now poised to be integrated into the health care system.

The 2012 Ithaca College Gerontology Institute Annual Conference will examine these changes, offering perspectives from the local, state, and national level, about how these trends will impact older people and the practitioners who serve them. Sessions will explore the opportunities and challenges associated with changes in Medicare and Medicaid, clinical integration, mental health services, and care transition models.

This one-day learning opportunity is aimed at helping health and human services practitioners understand these changes; learn about successful change strategies are being implemented in their communities; and gain knowledge about how these changes may affect their own work.

We are no longer accepting registrations for the conference.. Please see our message on the right side of this page for further information.
