INDIVIDUALIZED FLUENCY PROFILE
(Developed by Douglas E. Cross, 1996, 1997)

Instructions: Use the information obtained from the interviews, analysis of speech samples, and other evaluation tools to complete The Fluency Profile. Place a 1, 2, or 3 before statements that reflect the client's communication pattern. A ranking of "1" indicates the pattern is exhibited occasionally, "2" indicates it is exhibited frequently, and "3" it is exhibited almost always. Leave the statement blank if the client seldom or never exhibits the pattern. Transfer the appropriate values obtained during analysis of speech samples in the appropriate spaces under SEGMENTAL CHARACTERISTICS.

ANTICIPATION OF DISFLUENT/STUTTERING EPISODES
_____ Thinks about or anticipates future disfluent/stuttering moments
_____ Accurately predicts stuttering episodes (>70% )
