To submit a request for a Course Evaluation, a request form with the appropriate setup file/template indicated on the form must be provided for each evaluation.
Course Evaluations are used by departments to survey students about their courses. Course Evaluations are submitted to the DIIS Service Desk through the administrative staff of each department and their respective school.
All Course Evaluations require a setup file/template. Departments making changes to a current course evaluation should contact Computer Operations prior to submitting them for processing. A New/Change Template request form and a hard copy of the updated course evaluation must be provided. In order to create a new course evaluation setup file/template, a New/Change Template request form must be completed two weeks in advance to allow time for creation and to test compatibility with the system.
Instructors doing classroom surveys and/or research requiring processing should contact Computer Operations in advance. In order to create a survey or research setup file/template, a New/Change Template request form must be completed two weeks in advance to allow time for creation and to test compatibility with the system. A hard copy of the survey or research must also be provided.
Blue or Green Answer Sheets
Blue or green answer sheets are used by students to take a course evaluation. It is important to note that each course evaluation request may only contain either blue or green answer sheets that match the setup file/template. A combination of blue and green answer sheets may not be used.
Custom Answer Sheets
If a school or department would like to develop a custom answer sheet it is recommended they contact Computer Operations. Computer Operations will work with the school and vendor to develop a custom evaluation that will integrate with our system.
Student Answer Sheets
Answer sheets do not require student information such as student names or identification numbers.
Answer sheets must include the following:
- Name field:
- Instructors Name (e.g., Weeks K)
- Identification Number field:
- The course number should be used (e.g., 90000-01
- Sex, Grade or Educ, Birthdate, Special Codes fields: Typically left blank.
