Instructions: Enter information pertaining to the transaction you would like created. This form is to request moving funds that involve one or more 02 fund accounts. Information in this request will be sent to Lynn Eastman in Financial Services.
Do not use this form for Budget Transfers involving only the 01 fund accounts, instead, contact Noelle Bartolis in the budget office.
