IC/Longview Activity Request Form

This form is to be used by Ithaca College students, faculty, and staff who would like to propose volunteer or course-related activities at or involving Longview.

As forms are received, requests will be reviewed and a response will be made in a timely manner.

Please be aware of the following:

• All proposals will be receive careful review, but there is no guarantee that they will be approved.
• Proposals involving photography or filming will require additional documentation and review that can take up to two weeks. Only one film/photography project will be conducted at Longview each semester.
• The last day that proposals may be submitted for review and consideration is Monday, April 8th. (For proposals involving photography or filming, this deadline is Monday, March 18th.)
