HSHP Undergraduate Independent Study/Readings/Research Form

This form should be used by Undergradaute Students in HSHP who plan to complete an Independent Study/Readings/Research project with a faculty member.  In order to complete an independent project, the student needs to identify a faculty member willing to serve as a sponsor/mentor.  Students will be registered in the appropriate course once the petition has been signed and approved.
