This is a reminder that April 1st is the due date for completing Technology Renewal Summer 2013 questionnaires, which have been sent to all individuals, labs managers, and area coordinators.

You can also access the questionnaire via the Technology Renewal web page.  Select "Your Computer Assets" on the right side of the page and log in with your IC Netpass username and password.

Please take a few minutes to also verify the accuracy of the information listed, and submit any changes such as room number or guardian name.

It is important that we have all questionnaires completed by April 1st. 

If you are submitting a change request that generated a chargeback form, we also need to have the completed and approved forms submitted to ITS by that same date.
