 Please use this form to propose your ICSM and apply for Themes and Perspectives Designations.  You will email this form to Shaianne Osterreich and the deadline is December 3, 2012.  You should also submit the Intent to Teach form, with appropriate signatures, to your Dean's office by December 3rd.
