Funding Request DIRECTIONS

A Funding Request Form must be submitted BEFORE the directions below are applicable.

 

  DO NOT PURCHASE ITEMS BEFORE OBTAINING APPROVAL.

 

Travel-related: For study abroad and international travel, DO NOT submit the Travel Authorization Form (TAF) to Travel Services.   Submit the TAF to OSG only.

 

Document Checklist

 

 

Kaplan Test Preparation Course Fee (i.e., GRE, MCAT, LSAT, GMAT)*  

  1. First, fill out the Funding Request.
  2. If approved, you will receive permission to enroll from OSG Director with further instructions.
  3. NOTE: Fees may vary if enrolling outiside of Ithaca, NY; instruction will accompany email confirmation.
  4. Contact the Kaplan Education Center (email  stephanie.miller@kaplan.com) – carbon copy okittrell@ithaca.edu .
  5. Inform Kaplan of your CSTEM/HEOP participation (see Kaplan fees: /osg/docs/kaplan_fees/).
  6. Deposit may be paid by HEOP/CSTEM.  You may be responsible for any remaining fees.
  7. You will receive notification by OSG Director to enroll and the payment terms will be included.

 

 

Graduate School Exam Fee (i.e., GRE, MCAT, LSAT, GMAT, etc.) and Professional Membership Fees/Courses (i.e., APA, APTA, ACSM, NABA, etc.)

  1. If approved, you will receive permission to enroll from OSG Director with further instructions/permission to register and/or pay from OSG Director.
  2. Provide a receipt to me for reimbursement after you have registered.

 

Graduate/Professional School Application Fee* 

  1. If approved, you will receive permission to enroll from OSG Director with further instructions/permission to register and/or pay from OSG Director.
  2.  You should complete the application, pay the application fee, and request reimbursement, or
  3. Save the application and make an appointment with me to process payment of the application fee

 

Summer/Winter Tuition* (see below) and read Summer Aid

  1. Provide proof via an email/note/letter/doc that you requested to be considered for financial aid through SFS.
  2. If approved, you will receive permission to enroll from OSG Director with further instructions/permission to register and/or pay from OSG Director.
  3. Provide a copy of the course syllabi OR a course description from the current IC catalog
  4. Course tuition listed or anticipated tuition cost
  5. As applicable, copy of notice of acceptance/registration or other verifying document 

Conference fee(s)* 

  • Obtain permission from OSG Director to attend, register, and or travel
  • Other than NCUR 2013 , provide all conference-related materials and any http links
  • Once approved, as applicable, make an appointment with me to register and pay for the fees

 

IF Internship / Research / Field Experience / Externship or Job Shadowing Stipend* 

The site supervisor or professor* must provide a detailed email or note (on official letterhead) that states all:  

 

  • Acceptance notification to engage in this experiential learning or research opportunity
  • Length of this experience or participation dates. How many hours in total (estimate) will be completed?
  • List of non-clerical duties you will be performing or description of observation (i.e., job shadowing)
  • How this experience will enhance or improve your entry into a graduate program or STEM field/profession
  • If applicable, details of any monetary compensation provided for completing this experience

 

Note: If you are seeking a stipend for a course which includes a field experience, clinical practicum, or research component, provide a copy of the syllabus or other document that describes the nature of this experience. 

 

*Verification emails/notes/letters from undergraduate/graduate students or interns are unacceptable.

 

If your experience or course is credit-bearing through Ithaca College or elsewhere, please provide proof via an email or note from Student Financial Services (Julie-Hickman-Godoy) that you requested to be considered for financial aid through SFS.  This is required before your application will be reviewed and considered complete. 

 

 

NOTE: a course grade, evaluation, and/ or presentation may be required upon conclusion of this experience

 

 

 

Once your application materials are received, they will be reviewed, typically within 10-15 business days for further processing.  It is strongly suggested that you submit the Funding Request Form 4-6 weeks in advance of the applicable start date.

 

You will be notified via email when your application is approved or denied.
