United States: Bureau of Citizenship and Immigration Services (successor agency to the old INS).
Check out the laws passed by the US Congress since 1986 regarding immigration at the USCIS Public Law website as well as the specific Immigration regulations (click on "Title 8, Code of Federal Regulations (8 CFR)") that determine who gets into the US and how.
