Application and Payment Use this page to submit an application to the institute with a credit card payment of the deposit. If you don't want to apply and pay electronically, use the brochure request form to obtain a printed copy of the application form.. A complete application requires this form, a deposit of $100, your cassette tape, and a letter of recommendation. The first two requirements can be met electronically by submitting the form below with a credit card authorization for the deposit. However, your application will not be complete until we receive the cassette tape and letter of recommendation. All these materials must arrive by the April 1 deadline. So, please send the tape and recommendation right away. If you are accepted to the institute, we will bill you for the balance due.
