Emergency Contact and Supplemental Demographic Information

• Please enter your emergency contact information and/or your supplemental demographic information below.  Providing this information is strictly voluntary.
• All information (with the exception of directory information such as address and phone) is completely private and protected under the Family Educational Rights and Privacy Act (FERPA) and will not be shared with any parties outside the institution (including parents) without your written consent.  If you wish your parents to know your grades, you will need to provide them with that information either by sharing your HomerConnect ID and PIN or printing a copy of your grades yourself.  If that is not possible, we can send a copy of your grades to your parents.  However, you must request this in writing to the Registrar's Office each semester.
