Event Risk Assessment
To conduct a risk assessment on a proposed event, please complete the form below at least 30 days before your event. Please be comprehensive and descriptive with your responses in order to expedite the process. Risk Management and Public Safety will review your event details and contact you to discuss it further. If there will be an agreement or contract associated with this event please refer to the Ithaca College Contract Policy, This form is typically used for events on campus, if there is travel involved with your event please use the Domestic Travel Questionnaire.
