If you would like to pay by check, please download the form below. Please fill out a separate registration form for each camp you plan to attend. One health form may be sent for multiple camps.

Full payment and a completed health form must accompany each registration form and must be received two weeks before the start of camp. Registration forms not accompanied by the health form will not be processed. Registration forms received after the deadline are accepted on a space-available basis only, by permission of the camp director, and are subject to a $25 late fee. 

Please note that the Medical Screening Form should not be sent in advance. This form must be completed by the parent or guardian on the first day of camp.
