Documentation - LD


In order to ensure the provision of reasonable and appropriate accommodations for students with learning disabilities, the following documentation is needed:

  • A complete and current (preferably within five years of starting college) psycho-educational evaluation, which clearly states a specific learning disability (designated DSM-IV-TR diagnosis).
  • An individually administered aptitude test with all standard scores and sub-test scores included. Acceptable tests include:

· Wechsler Adult Intelligence Scale-WAIS-IV
· Stanford Binet Intelligence Scale (4th. Ed.)
· Kaufman Adolescent and Adult Intelligence Scale (KAIT)
· Woodcock-Johnson Psychoeducational Battery-Revised (WJ-III Normative Update)

  • NOTE: The Slosson Intelligence Scale-Revised, the Wechsler Abbreviated Scale of Intelligence (WASI), and the Kaufman Brief Intelligence Test (K-BIT) are not acceptable instruments.
  • A comprehensive achievement battery which includes all sub-tests, standard scores and percentiles reported on sub-tests administered, and current levels of academic functioning in reading, math, and written language. Acceptable instruments include but are not limited to:

· Woodcock-Johnson Psychoeducational Battery-Revised (WJ-R ACH)
· Wechsler Individual Achievement Test (WIAT)
· Stanford Test of Academic Abilities (TASK)
· Scholastic Abilities Test for Adults (SATA)

  • Specific areas of information processing (e.g. short and long term memory, sequential memory, auditory and visual perception/processing speed) must be assessed and discussed. Use of subtests from the WAIS-R/III or WJ-R COG is acceptable.
  • Functional limitations and recommendations for appropriate accommodations as substantiated by assessment and diagnosis should be addressed in the report.
  • Diagnostic report must be typed and include name(s), license number(s) of the qualified professional evaluator(s), and date(s) of testing.
  • Copy of latest IEP or 504 plan (optional.)
