During the clinical education course

1. Arrange for midterm contact.

Review the call/visit plan with your CI then confirm with the Department of Physical Therapy.

2. Complete a thorough orientation to the facility and department. Use form in featured documents box to guide orientation as needed.

3. Complete and return fax back form(s).  You and your CI should complete the same form.

4. (ongoing) Monitor progress towards learning objectives established in the course syllabus and towards established grading criteria.

5. Complete and review self-assessment on PT CPI  Web at midterm.

6. Plan, develop and deliver a mandatory inservice.

7. Complete and review final CPI.
