- Download a copy of the template, note the yellow highlighted sections and tip comments, upload final document to the registration system in the appropriate section of the documents tab.

Photo-Multimedia Release Form for Participating Minors
- Group Photo/Video: If you plan to photograph participating minors as a group (all participants are photographed), obtain a signature from all parents/guardians, in advance of the program or at time of registration. These signed forms must be uploaded in advance of the program.
- Small Group/Individual Photo/Video: If you photograph participating minors individually or in small groups (not all participants), obtain signatures only from the parents/guardians of those individuals photographed. These signed forms may be uploaded after the program concludes.
