  Important Forms to complete

• Return Disability Reporting Form and appropriate documentation to the Office of Academic Support Services for Students with Disabilities
• Return Health Certification Form and Meningitis Form to the Health Center by mail
• Complete Stafford Loan Entrance Interview online at www.ithaca.edu/finaid
• HEOP / IAP students should return paperwork to the Office of Multicultural Affairs
• Make sure official transcripts and AP scores are sent to the Office of the Registrar
• Complete and return Meal-plan forms and Health Insurance (or Health Insurance Waiver) forms to the Office of the Bursar
