Planned Studies Application
Before completing this Application, students interested in pursuing a Planned Studies degree (B.A. or B.S.) must first meet with the faculty coordinator of Planned Studies.
Download the form here, fill it out in Word, print it out to obtain the required signatures, and then submit the form to the Planned Studies Coordinator.
In addition to this form, applicants must also prepare and submit the following documents:
- a Change of Major/School form (submitted with this application)
- a Design Statement (submitted electronically)
- a Course List (submitted electronically)
- a letter of support from the Faculty Mentor (submitted either as hard copy or electronically directly to the Coordinator)
