Signing Up for a Membership

Details below regarding how to sign up and complete the new member process.
  • Fill out and submit the Medical History/Health Habit Questionnaire form to start the enrollment process!
    • Note: Submissions are saved in the Ithaca College Wellness Clinic Formstack HIPAA software account. All information is strictly confidential. 
       
  • Clinic professional staff will evaluate your needs and assign you a Fitness Specialist. We aim to be in touch within a week, based on demand and staffing. 
  • Submit any additional medical paperwork, if deemed necessary.
  • Fitness Specialist will initiate communication regarding the orientation process.
    • Paperwork (membership agreement & liability waiver) and payment (spouse/partner/community) will be handled during orientation #1.

  • Newly oriented member will be issued key tag after Orientation #2 and has full access to fitness floor, group exercise classes and services.
    • Additional orientation or personal training sessions are available with your Fitness Specialist. Contact the Clinic to schedule.

New Member Orientation Process

This process pairs a new member with a Fitness Specialist. More details provided below.
  • Meet with your assigned Fitness Specialist for a 30-60 minute Personal Training Consultation including Medical/Health History review, goal setting, and exercise program planning and 30 minute Fitness Evaluation. Please wear athletic clothing! 
     
  • Fitness Evaluation is conducted in person and lasts~30 minutes. With client consent and only if medically appropriate, Fitness Specialist measures resting heart rate and blood pressure, body composition (fat vs. lean tissue), muscular fitness, cardiovascular endurance, flexibility, and functional movement.
    • All evaluation measurements are voluntary. We appreciate client cooperation in providing a learning experience for our students.
  • Meet with Fitness Specialist virtually or in-person for Personal Training Consultation including Fitness Evaluation & GXT results review, finalizing goals, and introduction and orientation to your individualized exercise program. 
     
  • During this session, client will receive instruction in all cardio, resistance, and stretching exercises included in their exercise program. Please wear athletic clothing! 

Orientation #3-6 focuses on continuation and/or completion of workout session with Fitness Specialist observation, motivation, and feedback as needed. Please wear athletic clothing! 

Wellness Clinic Cancellation Policy

Please do your best to keep all scheduled reservations and appointments. We request 24 hours + notice for any necessary reschedules/cancellations.

Repeat cancellations may result in client placement on waiting list or in extreme cases membership suspension. Thank you for your understanding and support.
