In order to see if you qualify, fill out FYCSI consent form and email it to FCY4College@ocfs.ny.gov. Please include OAOA Assistant Director Sihaya Moraleda (smoraleda@ithaca.edu) on the email when you send the consent form.
Once eligibility is verified by State Education Department, you will receive a confirmation email from Sihaya Moraleda.
