How The Individual Choice Benefits Program Works
Individual Choice begins with a level of coverage necessary for the basic financial protection of each employee. This basic required coverage includes:
- medical coverage (unless you are covered under another medical plan)
- $50,000 of life insurance
- $50,000 of accidental death and dismemberment insurance (AD&D), and
- Long-term disability (LTD) coverage (once you become eligible)
You have the option to build upon these core benefits. With the Individual Choice program, you choose from an array of selections to meet your needs.
Summary Plan Descriptions
Summary Plan Descriptions (SPDs) are used to communicate benefit plan rights and obligations to participants and their dependents. The documents provide important information on specifics related to how benefit plans operate, eligibility, how services and benefits are calculated, when benefits become vested, when and in what form benefits are paid and how to file a claim and/or appeal for benefits.
