Any behavior that negatively targets an individual or group due to age, disability, race, color, national origin, religion, sexual orientation, gender identity or expression, marital status, or military status. You may use this form even if you are unsure whether your concern falls under this area.

Q: Will my report be kept confidential?
A: Only people who require information will receive it.
Q: What happens if the report I submit isn’t the right one for my situation?
A. We will route your report to the appropriate office.
Q: Should I be concerned about retaliation by the person(s) I am reporting?
A: The College has a strict policy against retaliation. While it is rare, it is also taken very seriously. If you experience retaliation, please report it so it can be addressed.
Q: Will my report automatically result in an investigation?
A: Not necessarily – whether an investigation is conducted will depend on the allegations and the level of information that is available. After filing a report, the person you meet with will explain the steps of a grievance process and all options available to you prior to initiation.
