To conduct a risk assessment for a proposed event, please complete the form below at least 30 days before your event. Please be comprehensive and descriptive with your responses in order to expedite the process. Risk Management and Public Safety will review your event details and contact you to discuss it further. If there will be an agreement or contract associated with this event please refer to the Ithaca College Contract Policy, This form is typically used for events on campus, if there is travel involved with your event please use the Domestic Travel Questionnaire.
Please note: If you are an OSEMA Recognized Student Organization who is hosting an event that involves risk, please schedule a meeting with Theresa Johnson, Assistant Director of Student Involvement. To schedule a meeting contact the OSEMA office at 607-274-3222; during the meeting Theresa will review the risk involved and assist with the completion of the Risk Management form.
