Below you will find links to all of the SAS forms, please check the respective webpages for more information and requirements. Filling out a form does not guarantee an accommodation, you must be registered with SAS and meet with a specialist.
Accessible Media Request
For students to alert SAS of inaccessible videos that were used in class.
Communication Access Survey
Designed to collect information about how you communicate and access information aurally.
Dietary Accommodation Request
Please use the "New Documentation form" on the SAS accommodation portal for any dietary/food related requests.
Disability Identification Form
Intended for students who are requesting accommodations through SAS.
Housing Accommodation Request
For IC students with a documented disability who require housing accommodations.
Materials in Alternate Format Request
For students who have been approved to receive alternate format materials as an accommodation.
Notetaker Request
For students who have been granted notetaking/class notes as an accommodation, used to request a peer notetaker.
Information Release Consent
Used to identify how your SAS information will be shared with parent(s) and/or guardian(s). Use link in the left navigation menu.
SAS Accommodation Plan Renewal
For renewal of SAS Accommodation plan and must be completed each semester. Not applicable for first year students.
