Media Productions - Insurance (COI)

Important - Please read.

Risk Management will ONLY issue a COI (certificate of insurance) when a specific request is made by the location owner/manager and when the following steps are completed, in order. 

Do not offer to provide evidence of insurance to any locations; a COI is provided only when specifically requested and after all requirements are met. Offering insurance when not required will delay your approval process.

  1. Submit the Form: Complete and submit the Off-Campus Film/Media Production Form.
  2. Faculty Review: Your professor will review the submission to approve or decline the production’s progress.
  3. Dean’s Office Review: Upload all location documents (insurance requirements, permits, or contracts) to your form.
    • Note: Students are not permitted to sign contracts or permits on behalf of Ithaca College. Only the Dean’s Office can sign these documents.
  4. Provide Insurance Details: Send Risk Management the specific insurance requirements from the location, including:
    • Entity name and full address.
    • Required insurance types and coverage limits.
    • Note: If "Additional Insured" status is required, you must coordinate with the Dean’s Office to involve Legal Affairs for contract review.
  5. COI Issuance: The Office of Risk Management will issue the COI only after this process is complete.

City of Ithaca Permits (Exception): 
You may submit a City of Ithaca film permit request without an attached COI. If the City approves your request, they will contact Risk Management directly to coordinate the insurance. Risk Management will then send the COI straight to the City. This streamlined process applies exclusively to City of Ithaca permits.
