We encourage you to review this resource page to verify shoot details and documentation requirements before submitting your form. Following these steps will help ensure a smooth and timely approval process.

Pre-Submission Checklist:

[ ] Faculty Approval: Has your instructor reviewed your script and approved your use of props/locations?

[ ] Lead Time: Are you submitting this at least 21 business days before your shoot?

[ ] Location Details: Do you have the full name and address for every location (up to 5)? Be prepared for each location to provide the specific activity/props.

[ ] Prop List: Have you identified every prop, including "high-risk" items like weaponry or drones?

[ ] Documentation: PDF copies of any owner agreements, municipal permits, or insurance requirements to upload?

[ ] Safety Plan: If using stunts, weapons, or special effects, is your detailed safety plan ready to be typed in or uploaded?

[ ] Natural Lands: If filming in the IC Natural Lands, have you reviewed the rules/guidelines and agree to abide by them?

[ ] Insurance Requirements: ONLY when a location or equipment owner requires proof of insurance, have you gathered the applicable insurance details or written requirements?

Pro Tips:

  • Shooting in an IC dorm, IC classroom, IC parking lot or IC green space, etc.? Use the ON-CAMPUS FORM.
  • Shooting in a local cafe, public street, the Commons, restaurant, a public park or private residence, etc.? Use the OFF-CAMPUS FORM.
  • Need a Certificate of Insurance (COI)? Be sure to note the insurance requirements page.  Tip: If a location requires "Additional Insured" status immediately notify the Dean’s Office, as this will require a contract/agreement that can take up to 4 weeks. (COI's are only applicable for Off-Campus shoots.)
  • Be mindful of normal business hours when reaching out via email or phone and allow appropriate time for responses outside of those hours.
