Please Note: A signed parental consent form for each minor participant must be uploaded to the document tab in the minors registration system for your program, under the appropriate document field. Consent forms can be saved and uploaded as single documents or grouped together in one document.

If the format or terms of the template do not appear suitable for your program, please contact the Office of Risk Management and/or the Office of Legal Affairs to modify them, as needed.

Roles and Responsibilities Information for Programs Involving Minors (R&R Document)
- Download a copy of the template, note the yellow highlighted sections in the template. Please refer to the Roles & Responsibilities instruction document which has helpful tips and alternative language to assist you in tailoring the template for your program.
- The only individuals identified by name, in the R&R Document, should be the Program Director and Co-Director.
- When you have a final document, please upload to the registration system in the appropriate section of the documents tab.
- A portion of the R&R document is provided to parents along with the Parental Consent Form, the full document should be provided to your program personnel during your orientation.

- Group Photo/Video: If you plan to photograph participating minors as a group (all participants are photographed), obtain a signature from all parents/guardians, in advance of the program or at time of registration. These signed forms must be uploaded in advance of the program.
- Small Group/Individual Photo/Video: If you photograph participating minors individually or in small groups (not all participants), obtain signatures only from the parents/guardians of those individuals photographed. These signed forms may be uploaded after the program concludes.
